1. Written Consultation
   Upon receiving relevant information from you (materials such as medical records, examination reports, X ray reports, or medical reports), our staff will forward them to the medical team. Then the medical team will provide a formal written Second Opinion Consultation Report for you regarding your condition(s).
2. Telehealth consultation
   Upon receiving relevant information from you (materials such as medical records, examination reports, X ray reports, or medical reports), our staff will forward them to the medical team. Then our staff will assist you in arranging a consultation time with our doctor(s).

In order to help us gain a better understanding of your needs, please kindly fill out the “Telehealth/ Second Opinion Application Form”, clearly stating your requirements, so that we may gain a better understanding of your medical needs.

FEES
Once your application is confirmed, our staff will provide an estimated cost of fees for this service. Please send the estimated fee to us.
1. Written Consultation:TWD $6,000 each person/case （USD 200/person/case)+ TWD $500 registration fee
2. Telehealth consultation:TWD $6,000 each person/case （USD 200/person/case)+ TWD $500 registration fee

NOTE
1. Each Teleconsultation is 30 mins in length.  An additional NT$1,000 will be charged for every additional 5 minutes.
2. If there is an excessive amount of X Ray, CT and MRI, then additional charges will apply.
Second Opinion Consultation fees will vary depending on each case and the actual conditions or specialty, therefore, above described fees are solely for reference only.

TELECONSULTATION REQUIREMENTS
1. Please prepare a camera-enabled computer, laptop, smart phone, or tablet, and ensure your internet connection status is stable. (WIFI connection is most recommended)
2. If you are using a smart phone or tablet, please download the “Cisco WebEx Meetings”, to allow video conference with the doctor(s).
3. After download the “Cisco WebEx Meetings”, our staff will send a link to you. Please click and connect through the link on the time and date that is arranged for you. Then you will be ready to consult with our doctor(s).
